Conflict Free Case Management Services
Conflict Free Case Management services are tailored to meet the individual's wants and needs. The case manager works in conjunction with the individual, family and/or guardian to design a person centered Plan of Care (POC) to ensure satisfaction of services via monitoring each support received and to document progress. The case manager also monitors all areas of health, safety and wellness.
Conflict free case managers also assist individuals in obtaining other SCL services such as:
Residential Supports
Residential Supports may be received in a staffed residence or with a family home provider. A staffed residence may service up to three individuals with supervision by a variety of trained staff. Family Home Providers are qualified families who provide appropriate levels of supervision and residential supports within their own home.
Respite
Respite services are provided for an individual that is living with a Family Home Provider or their own family. Respite may be arranged on an hourly or daily basis to provide relief for the primary caregiver.
Behavior Supports
Behavior Supports are services designed to assist individuals to develop positive interactions. The behavior specialist may develop a behavior support plan based upon the evaluation of the frequency and/or severity of identified problem behaviors.
Adult Day Training
Day services that are intended to support the participation of individuals in daily, meaningful and valued routines of the community. Services may include the development of social, daily living and self-advocacy skills and may be provided in a work-like setting or in the community to support individuals in obtaining their social, vocational or educational outcomes.
Supportive Employment
Supportive employment provides assistance in obtaining gainful employment. Job development, job training and on the job supervision will be provided by staff according to the needs of the individual.
CLS
CLS is a program for individuals that live with their family. It provides individualized care in the community based on the needs of the individual.
Speech, Physical & Occupational Therapies
These services may be provided when recommended by the primary care physician. Therapy will be provided by a qualified therapist.
